How do I request a copy of my medical records?
You can request a copy of your medical records by filling in the below form. You can then either email it, send it via post, or hand it in at the surgery.

We would also need to see a form of ID in order to process the request. We accept copies. If handing it in to the surgery, we can photocopy your ID here if you are unable to. You can also take a photo of the ID and send it via email with the application form.
