Registering with the surgery

To register with the surgery:

• download a new patient registration form (PDF) and:
  • Ffurflen Gofrestru Cleifion Newydd (Welsh- Word)
  • adult health questionnaire (Word) or
  • child health questionnaire (Word)
• visit reception when we are open and collect a paper registration form.

You can return these by either posting to us, emailing them or handing in to the surgery.
